The $99 payment includes a 30-minute phone consultation. Payment is required at the end of questionnaire. This information will automatically submit to our office and we will contact you to schedule the next step of the RejuvinAge Program. The Program Appointment, Lab Panel and Medications are not included in promotional pricing. Online payments for special offers are non-refundable. Appointments may be rescheduled with 72 hours’ notice.
