How to Get Started
To get started please complete the online patient questionnaire. Once submitted, you will provide the $99 payment via credit or debit card. This information is required to schedule your appointment providing Dr. Krup a detailed background on your health and lifestyle to best maximize the value of your first appointment.
To begin, simply select our Female Questionnaire or Male Questionnaire. Information is securely processed and completion takes approximately 15 minutes. Once your information is received, our Patient Coordinator will reach out for appointment scheduling.
